Geriatrics Medicine Program

The Geriatrics Medicine program is a specialized division that focuses on the care and treatment of elderly patients. The program offers a range of services, including:


  • Monitoring long-term treatment of chronic conditions
  • Medication management
  • Temporary visits after hospitalization
  • Coordination of care with home health agencies
  • Basic blood work, procedures, and imaging in the home
  • Coordination of social needs
  • Coordination with UTMB health specialists and hospital
  • Palliative Care

Eligibility Criteria

To be eligible for the Home Based Primary Care Program, patients must meet the following criteria:


  • Be over 65 years of age
  • Have difficulty transporting to clinic appointments due to cognitive or physical decline
  • Have a history of stroke, severe decline in mobility, Parkinson's, dementia, COPD, congestive heart failure, or oxygen dependency
  • Be a resident of Assisted Living or Memory Care Unit
  • Geographical restrictions may apply
  • Each case is reviewed by the Home Based Primary Care Call Team for acceptance
