Geriatrics Medicine Program
The Geriatrics Medicine program is a specialized division that focuses on the care and treatment of elderly patients. The program offers a range of services, including:
- Monitoring long-term treatment of chronic conditions
- Medication management
- Temporary visits after hospitalization
- Coordination of care with home health agencies
- Basic blood work, procedures, and imaging in the home
- Coordination of social needs
- Coordination with UTMB health specialists and hospital
- Palliative Care
Eligibility Criteria
To be eligible for the Home Based Primary Care Program, patients must meet the following criteria:
- Be over 65 years of age
- Have difficulty transporting to clinic appointments due to cognitive or physical decline
- Have a history of stroke, severe decline in mobility, Parkinson's, dementia, COPD, congestive heart failure, or oxygen dependency
- Be a resident of Assisted Living or Memory Care Unit
- Geographical restrictions may apply
- Each case is reviewed by the Home Based Primary Care Call Team for acceptance
