This course has been designed to meet the needs of registered and practicing professionals such as nurses, paramedics, physiotherapists, occupational therapists, and radiographers. The course aims to provide practitioners with the knowledge and skills to proactively support the holistic care and treatment of individuals with long-term health and social care needs. It will focus on enabling individuals to take greater control of their care, offering opportunities for better health and wellbeing, through personalised care planning, increased prevention strategies, and supported self-care.

How You Study

This course is delivered through blended learning, and includes workshops which will focus on the development of practical and collaborative inter-professional skills and discussion-based seminar groups which are supplemented by directed learning. Online evidence-based resources will be made available to support this approach. Students will also be expected to work collaboratively with professional colleagues in their service area to ensure that they have the advanced skills and competencies to proactively manage individuals requiring holistic care for a long-term condition within their professional service area. Outside of the classroom, students will also be encouraged to further their own studies and contextualise their learning to their own service area through independent study.
