Fire Safety & C-14 Certificate of Fitness

The C-14 Certificate of Fitness is required for non-production chemical laboratories. To apply, individuals must:


  • Attend a training session given by the EHOS team
  • Submit required documents, including:
    • Confirmation of C-14 training and other lab safety training attendance
    • Color copy of CCNY Employee (Faculty/Staff) ID card
    • Recent photo (2x2 headshot)
    • Color copy of diploma/transcript showing degree conferred
    • Recommendation letter from an advisor or principal investigator
    • Fee waiver letter
    • C-14 Alternative Issuance Procedure - Employee Affirmation Form (must be notarized)

Certificate of Fitness Renewal

To renew the C-14 certification, individuals must follow the instructions in the C-14 Certificate of Fitness Application Guide. The renewal process does not require additional training sessions.
