Fire Safety & C-14 Certificate of Fitness
The C-14 Certificate of Fitness is required for non-production chemical laboratories. To apply, individuals must:
- Attend a training session given by the EHOS team
- Submit required documents, including:
- Confirmation of C-14 training and other lab safety training attendance
- Color copy of CCNY Employee (Faculty/Staff) ID card
- Recent photo (2x2 headshot)
- Color copy of diploma/transcript showing degree conferred
- Recommendation letter from an advisor or principal investigator
- Fee waiver letter
- C-14 Alternative Issuance Procedure - Employee Affirmation Form (must be notarized)
Certificate of Fitness Renewal
To renew the C-14 certification, individuals must follow the instructions in the C-14 Certificate of Fitness Application Guide. The renewal process does not require additional training sessions.
