About Program
Program Overview
This inter-professional module will provide professionals from health, education, and social care disciplines with essential knowledge and skills to provide holistic, inter-professional care to children and young people with life-limiting conditions and their families. Practitioners will be supported to develop a toolkit of resources that will enable them to provide care to life-limited children, young people, and their families. This will include discharge planning, caring for the client group across different settings, and taking account of the holistic needs of the child or young person and their family e.g. counseling, bereavement, and spiritual support.
