What You Will Study

The successful achievement of this skill set requires you to complete all units listed below. You will also be required to undertake 100 hours of vocational placement in an approved setting.


Core Units

  • CHCCSM012: Coordinate complex case requirements
    • This unit describes the performance outcomes, skills and knowledge required to coordinate multiple service requirements for people with complex needs within a case management framework.
  • CHCCSM013: Facilitate and review case management
    • This unit describes the performance outcomes, skills and knowledge required to plan, monitor and review case management.
  • CHCCSM016: Undertake advanced assessments
    • This unit describes the performance outcomes, skills and knowledge required to undertake initial and ongoing assessments with a person, to determine eligibility, priority and need for services as part of a case management process or as part of an assessment service.

Entry Requirements

  • This is NOT a skill set for those wishing to move into Case Management, it is designed to develop current Case Management skills and knowledge.
  • This skill set has been endorsed by industry as suitable for individuals who:
    • hold a qualification at Certificate III level or higher in community services or health or related field with 1 - 2 years experience in case management, demonstrated through a current resume.

Resources Required

  • It is recommended that you have access to a reliable internet connection to access TAFE Queensland's online learning system Connect, and a computer with up-to-date software, including Microsoft Office and Adobe Acrobat Reader.

Placement

  • During your course you will be required to complete 100 hours of vocational placement so you can start practising your new skills in a real-world environment.
  • You will be expected to manage family and work commitments in order to complete your shifts.
  • You are required to complete 100 hours of placement to satisfy this component of your qualification.

Important Information

  • This course requires you to undertake role plays. Role plays will cover sensitive topics such as trauma, domestic violence, cultural diversity, mental health, and children at risk.
  • You are expected to engage in additional personal study to meet the course requirements.
  • Each assessment task will have specific criteria that you need to meet, which will be clearly outlined in your assessment instructions.
