Overview of the Chronic Cardiac Disease Management Micro-credential
The Chronic Cardiac Disease Management micro-credential is designed for registered nurses working in hospital or community settings who care for patients with chronic cardiac conditions or those at risk of developing this condition. The aim of this micro-credential is to enhance knowledge and competencies in caring for individuals living with chronic cardiac disease, facilitating a shift from a disease-focused to a more person-focused model of care.
Admission Requirements
To be eligible for the Chronic Cardiac Disease Management micro-credential, applicants must:
- Hold a level 8 (undergraduate) degree in nursing or a related field.
- Alternatively, satisfy the course committee that they can successfully complete the micro-credential program based on evidence of experience working in a healthcare setting with patients or service users with chronic cardiac disease and evidence of recent Continuing Professional Development (CPD).

What Will I Learn?
Upon completing this 10 ECTS micro-credential, learners will be able to:
- Discuss the impact of risk factors on the pathophysiological processes contributing to cardiac disease development.
- Demonstrate the requisite knowledge and skills in health promotion in relation to cardiac disease.
- Compose and critique an action plan in collaboration with the person living with a chronic illness that is person-centered and evidence-based.
- Demonstrate the knowledge, skills, and attitudes required to conduct a motivational interview with a patient/service user.
Course Delivery and Assessment
- The micro-credential will be delivered in a blended format (in-person/online) with 4 days in-person and 2 days online.
- Assessment includes a Case Study Presentation (100%) and Making Every Contact Count, with e-Learning and Workshop components assessed on a pass/fail basis.

Program Structure
The program involves engaging in online directed and self-directed learning activities, participating in discussions on critical issues and peer-to-peer learning groups, composing an action plan based on a person in your care living with chronic cardiac disease, and appraising your own action plan. The micro-credential includes activities related to the program, such as completion of eLearning in the Making Every Contact Count (MECC) program, and group work in discussion of action plan implementation to facilitate peer learning following theoretical content provided during the course.
