Programme Overview
Chronic diseases are long-term medical conditions that are generally progressive. Statistics show that Singapore’s ageing population and adoption of unhealthy lifestyles have contributed to the increasing prevalence of chronic diseases.
This course is designed for healthcare professionals caring for patients with chronic diseases in the community setting.
Aims of Programme
This programme allows participants to:
- identify drivers and challenges shaping the healthcare environment;
- explain the characteristics of common chronic diseases;
- describe the clinical manifestations, medical, nursing and pharmacological management of common chronic diseases;
- discuss self-management strategies for clients with common chronic diseases;
- describe available support for clients with common chronic diseases and their carers;
- apply motivational interviewing skills when interacting with clients;
- apply effective strategies in health education and health promotion; and
- recognise the importance of empowering patients to self-manage chronic conditions.
Programme Details
- Duration of Programme: 5 full days from 9.00 a.m. to 5.00 p.m.
- Who Should Attend: Any healthcare professional involved in care delivery of clients with chronic diseases.
