Program Overview


Diabetes Prevention and Education Program

The Diabetes Prevention and Education Program is designed to help patients manage their diabetes. The program includes tips for healthy eating, being active, monitoring blood sugar, taking medicine, and reducing risks.

The program is run by certified diabetes educators who help patients develop self-management skills, achieve better metabolic control, improve lipid levels, and reduce blood pressure.


Diabetes Prevention Program (DPP)

The Diabetes Prevention Program (DPP) is a comprehensive program that helps patients prevent or delay the onset of diabetes.


Diabetes Self-Management Training (DSMT)

The Diabetes Self-Management Training (DSMT) program teaches diabetes patients how to cope with and manage diabetes. The program includes:

Program Details

  • New patient visits are 60 minutes
  • Return visits are 30 minutes
  • Group sessions are planned for 60 minutes
  • Due to the pandemic, we are currently not offering group classes
  • DSMT is an individualized care plan so topics can vary every session
  • DSMT includes a comprehensive diabetes educational needs assessment pertaining to the following content areas:
    • Glucose monitoring
    • Physical activity
    • Healthy eating
    • Taking diabetes medications
    • Problem solving
    • Healthy Coping
    • Risk reduction
  • Our certified diabetes educators are trained in all commercially available insulin pump systems and continuous glucose monitors (CGM)
  • People with diabetes who are interested in learning more about diabetes technology should discuss this during their DSMT visit
  • If you are diagnosed with Type 1, Type 2, or Gestational Diabetes Mellitus, we highly encourage you to schedule a visit
  • Depending on insurance plan, there may be a copayment
  • Most patients do not pay more than they would if they saw their doctor or specialist
  • If you believe that a friend or relative can benefit from DSMT, please encourage them to talk with their provider about getting them a referral
