Practice Innovation Program

The Practice Innovation Program is a comprehensive initiative aimed at transforming primary care practices through innovative approaches and evidence-based methods.


Introduction

The program introduces a range of initiatives, including the 1302 Behavioral Health Integration, PREPARE 4 CGM, Integration of Perinatal and Substance Use Care, Primary Care Diabetes Lab, Implementation Support for MOUD, Glycemic Excursion Minimization/Glucose Everyday Matters (GEM) Supplements, and Diabetes Distress.
